Pam Outen Big Smile Award Nomination Form
Outstanding Junior 4-H'er of the Year: 4-H'ers ages 9-12 have the chance to receive this great honor! Tell us what you have done in 4-H, and you may be one of 2 4-H'ers awarded at our annual Achievement Program. Forms due in January.
Outstanding Senior 4-H'er of the Year: This honor goes to two 4-H'ers ages 13-18 who have showed excellent performance in their 4-H career. Award winners will receive a 4-H Scholarship. Forms due in January.
4-H Project Completion Club of the Year: Leaders, as your club turns in those project books, record them on this form. Turn it in in January to compete for this award.
4-H Community Service Club of the Year: Using this form, record your club's community service projects. At the Achievement Program, one club will be recognized for their outstanding contributions to the community. Forms due in January.
Leader Recognition Form: Leaders, you may fill out this form for yourself, or 4-H'ers, you can nominate your favorite leader. Once a year, we select our Cabarrus County Leader of the Year. This honor is presented at the Achievement Program. Forms due in January.

Medical Release: Leaders and 4-H staff need this form filled out for every 4-H trip or event that you participate in. You will not be able to join us if we do not have this document.
Code of Conduct: By signing this agreement, 4-H'ers understand what is and is not allowed during 4-H events. We must also have this document for youth to join us at any events.
